Care Journey
At Venus we provide a unique proposition of being able to provide care, support and accommodation throughout a person’s full care journey. We achieve this by establishing a close partnership with social landlords who can adapt to our requirements, be it a specific location, a type of property or layout of the property. This provides stability of the accommodation without risk of short term relocations. Our organisation is set up to allow maximum flexibility for any changes in support levels, whilst maintaining continuity of care and familiarity of a constant staff mix
Continuity of Care
There is a continuity of care within these care levels where someone can be stepped up or down along their care journey as their needs change ensuring any movement across the 3 tiers within Venus is seamless. We strive to dedicate a Team Leader and staff team for each Service User ensuring that any progress made are not lost on transitions and they remain stable, reducing any incidents of behaviours that challenge.
Within our Medium Intensity category, we provide Community In-Patient Step-Up/Step-Down Units and Transitional Living Facilities. These services offer flexible support pathways:
- Step-Down placements for individuals discharged directly from Assessment and Treatment Units (ATUs) or our Crisis Facilities who require continued structure and therapeutic support before moving to more independent settings.
- Step-Up placements for individuals experiencing a deterioration or crisis at home, offering a safe and supportive alternative to hospital admission.
Within our Low Intensity category, we provide support through our Long-Term Independent Living Placements. Where individuals express a preference for a particular location – for example, wishing to settle closer to family, friends, or a familiar community – we are able to source suitable properties that meet their needs and aspirations.
All care plans are bespoke and tailored for individual needs. A significant number of our residents are diagnosed with mild to moderate or severe learning disabilities. This is common to other diagnoses we treat but is especially prevalent with residents diagnosed with autism or Down’s syndrome.
Care plans are carefully structured and tailored to respond to the needs and behaviours of each individual, which includes speech therapies, sign language, pictorial evidence and other means of communicating.
All communication and therapeutic interventions are focused on promoting the individual’s ‘activities of choice,’ while de-escalating any anxiety or stressful situations that may arise.
